1. Your health care insurance coverage may have been discussed before you arrive, but please bring your health insurance ID card.

 

 

2.          A list of all prescription or non-prescription medications you have              recently taken or are taking now.

 

 

3. For your convenience, we have our welcome documents here, which you can open, print and sign, to bring in when you have your appointment. (Fill in what you can and we will do the rest)
